Held-item equipment handover
HC-HAND-01 / A CONCEPT
Held-item equipment handover
DRAFT — educator and biomedical review required
HCM-T01 / HC-CFG-01-A-CONCEPT
Practise closed-loop communication about an unresolved equipment issue, using synthetic training roles and no patient information.
01. Confirm the same configuration [BODY]
Identify HCM-T01 and the source/configuration revision before the conversation.
Evidence: The same asset is named.
STOP: Unknown identity: stop and clarify.
02. State the held condition [TAG-H]
Say that the training station is HELD for educator review. Preserve the restriction without implying a repair or clinical release.
Evidence: HELD state stated clearly.
STOP: Do not soften or omit the restriction.
03. Name the discrepancy [C-B]
State expected C-A and observed C-B. Describe the unresolved accessory/source mismatch without attempting a fit.
Evidence: Expected and observed identities distinguished.
STOP: Do not present C-B as a usable substitute.
04. Attach the source [NOTE]
Name HC-MON-01 A CONCEPT or the approved replacement source and the missing evidence.
Evidence: Source and evidence gap named.
STOP: No invented reading, approval or completion record.
05. Choose the receiving role [NOTE]
Identify the educator or designated review role and agree the next action. Use synthetic roles in the rehearsal.
Evidence: Next owner and requested action are clear.
STOP: An unaccepted handover remains open.
06. Ask for repeat-back [NOTE]
Receiver repeats the asset, restriction, discrepancy and next owner.
Evidence: Repeat-back observed in the training exercise.
STOP: Missing restriction: correct and repeat.
07. Confirm the message [NOTE]
Sender confirms or corrects the repeat-back. Keep an equipment-only training acknowledgement.
Evidence: Closed-loop acknowledgement complete.
STOP: No patient data or real employee scoring record.
08. Keep status separate [TAG-H]
Review communication quality while retaining HELD equipment status. A separate authorized process resolves equipment availability.
Evidence: Learning result and equipment restriction kept distinct.
STOP: A good handover does not constitute equipment release.
Source basis: Existing HCM-T01 scenario and continuity bible; no approved operating SOP supplied.
A completed rehearsal never releases equipment for clinical use.
